PLEASE READ CAREFULLY BEFORE SIGNING. THIS DOCUMENT IMPACTS YOUR LEGAL RIGHTS.

By completing this form, I agree to the following terms on behalf of myself and/or any minor children listed below:

1. Assumption of Inherent Risk: I acknowledge that participation in inflatable attractions, bounce houses, and interactive play at The Harvest Hop Bounce House involves physical activity that carries inherent risks, including but not limited to slips, falls, collisions with other participants or equipment, cuts, bruises, sprains, or other bodily injuries. I voluntarily assume all risks associated with participation.
2. Rules and Safety Compliance: I agree to follow all posted safety rules, staff instructions, and equipment guidelines at all times. I agree to supervise any minor children under my care while on the premises.
3. Release of Liability: I hereby release, waive, discharge, and hold harmless The Harvest Hop Bounce House its owners, operators, employees, and agents from any and all liability, claims, demands, or causes of action arising out of negligence or participation in any activities on the premises.
4. Digital Signature Agreement: I certify that I am at least 18 years of age and the parent or legal guardian of any minor participants listed. Entering my name below serves as my legal digital signature and confirms my full agreement to these terms.
